NHS Blackburn with Darwen Clinical Commissioning Group (CCG) is currently looking for feedback from diabetes patients or their carers about the diabetes care they have experienced in their GP practice.
The CCG, the commissioners of the service provided, is currently looking at re-designing the diabetes service in Blackburn with Darwen with the aim of bringing more care closer to home.
Lisa Kiernan, Head of Primary Care and Integrated Community Services at NHS Blackburn with Darwen CCG, said: “As a CCG, we are committed to meeting the needs of patients and constantly striving to improve service user experience. Our aim is to provide patients with the highest standard of care possible.
“If you suffer from diabetes or care for someone who does, and you are registered with a GP in Blackburn with Darwen, then we would like your feedback about diabetes care experienced in your GP practice. Your responses, along with others, will help to identify any areas we can improve upon in the future.”
A questionnaire can be picked up from your GP surgery or download from the CCG website at http://www.blackburnwithdarwenccg.nhs.uk/get-involved/diabetes-survey/ Alternatively, this can be completed by telephone.
The CCG is also looking to hold a focus group to allow them to hear first-hand from those with experience of living with diabetes, either as a patient or carer. This insight will then be used to help support the care available to those in the community experiencing diabetes.
