Integrated Care is the approach being taken within Blackburn with Darwen to improve your health and social care services. Integrated Care will bring high quality, effective, safe health and social care to our residents. It means supporting people in the community, through a personalised approach, to better care for themselves, with the help of their friends and family, to manage health conditions and changes in health and social circumstances.
Integrated Care will see local teams of GP’s, practice nurses, social workers, mental health workers, community nursing teams and other health professionals including pharmacists working together to plan people’s care. The teams will be aligned to four localities within Blackburn and Darwen: North; West; East, and Darwen. Coordinating the services of health, social care and the voluntary, community and faith sector organisations will ensure all residents have access to the same outcomes irrespective of where they live. The social care and community health services will be built around GP practices, to deliver care closer to peoples’ homes and provide alternatives to hospital based care where appropriate. A Case Manager will work with people identified at risk, their carer(s) and other health and social care professionals to develop a personalised care plan, and continue to monitor the needs of the patient to coordinate their care, promoting independence and self-care where appropriate.
Integrated Care incorporates Intensive Home Support, also described as hospital at home, which is an intensive care programme that offers an alternative to long term residential and nursing care. A medically-led multidisciplinary team will focus on patients most at risk of hospital admission, initially the frail and the elderly.
