A new service to support vulnerable people who frequently attend Royal Blackburn Hospital Emergency Department (ED) is due to be launched in July.
The Service which has not yet been named; is aimed at supporting those patients who regularly attend the Emergency Department due to the complex nature of the issues they face, these patients often have a mixture of physical, social and mental health issues
The integrated service will work closely with the ED and positively impact on the time these patients were waiting for support and free up capacity in the wider ED service / North West Ambulance and hopefully reduce hospital admission due to timely interventions for this group of people.
Service Users where possible will be deflected from ED and be assigned a case worker’ to provide short to mid-term intense professional/ peer support to help resolve the often distressing situations which compound many physical and mental health difficulties including problems with; housing, debt, health and wellbeing, domestic violence, substance misuse, loneliness and isolation. ….
Once stable, Service Users will be supported to continue to engage in Community Services and this may include supporting registration with a GP practice and other support networks for any ongoing health/ support needs.
Blackburn with Darwen Clinical Commissioning Group (CCG) and East Lancashire CCG have awarded the contract for the new service to Change, Grow, Live (CGL) following a new interactive engagement process with a number of providers that lasted several months.
The Commissioners wanted to appoint the most capable provider who would then be able to co-produce the specification for this new innovative service alongside Service Users, third sector providers, the CCGs and Local authorities.
The new service will provide an integrated solution to bring together a number of separate liaison services that provided in reach into the Emergency Department at the Royal Blackburn Hospital including the hospital alcohol liaison and police liaison service. This will help reduce overlap and duplication between services and ensure that the patient receives a joined up single approach to help improve their outcomes whilst providing more capacity in the wider ED.
